Chung Wah Community Care (Chung Wah CC) recently hosted an informative "Home Care and Aged Care Services" seminar at the Citiplace Community Centre near Perth train station. It was a great success attracting over 30 community members and spearheading the CWCC care finder program for the multicultural community. Chung Wah CC is a proud member of the initiative that is led by the WA Primary Health Alliance, offering free care finder services to the northern Perth community.
Funded by the Australian Government's Primary Health Network Program, the care finder program aims to support seniors in need of assistance to access aged care services. Our care finder team is dedicated to guiding you through the complexities of aged care services. We're here to help you understand the relevant information, prepare the application, undergo evaluations, and assist in selecting the most suitable service providers. Our team can also help you in finding and transitioning to a new, better-suited aged care support if you are already receiving aged care services.
The services provided by our care finder team at Chung Wah CC include but are not limited to:
– Providing translation services to facilitate communication.
– Representing applicants in communicating with My Aged Care and arranging evaluations.
– Participating in evaluations and providing support to applicants.
– Assisting in the search for aged care service providers in the applicant's area and helping to screen based on their requirements.
– Assisting applicants in filling out forms and understanding service agreements.
– Following up after the applicant has received care services, and
– Assisting seniors in dealing with other life difficulties and getting related help, such as physical and mental health problems, housing difficulties and homelessness, substance and alcohol abuse, and referrals to social group services.
