On December 10, 2024, Chung Wah Community Care (CWCC), in collaboration with Carers WA, hosted an informative session titled Pathways to Post School Life at the Willetton Community Centre. This event, part of the Carer Gateway initiative, featured expert speakers from the NDIA and Mission Australia, offering practical transition support for young individuals nearing school completion and their carers.
Graduating from school marks a significant transition filled with opportunities and challenges, particularly for young people with disabilities, their families, and carers. During this period, many families face uncertainties about the future and the complexities of planning. By understanding and utilising the support provided by the NDIS (National Disability Insurance Scheme), families can significantly alleviate stress and prepare effectively. These supports include personalised transition planning, access to education and training, employment assistance, and resources for home and community living.
The session also covered essential preparations for adulthood, such as changes in legal rights, financial management, health care arrangements, and strategies for achieving independent living. Attendees learned about psychological support services and training for carers provided by the NDIS, which are designed to empower families to navigate the transition with confidence and ease.
In CWCC’s safe and professionally supported environment, carers had the opportunity to connect with others who share similar experiences, exchanging stories, insights, and practical advice. This supportive atmosphere, enriched by shared language and cultural understanding, allowed participants to express their emotions freely, gain valuable guidance, and experience deep emotional comfort.
