Shining a light on essential aged care needs, an insightful talk recently graced the heart of Perth.
On 18th August, the care finder team from Chung Wah Community Care (Chung Wah CC) held an educational talk on "In-home Aged Care & Services" at the Citiplace Community Centre near the Perth Train Station. This event forms an integral part of the care finder program and has hosted over 20 participants from the community. Chung Wah CC is now part of the care finder initiative spearheaded by the WA Primary Health Alliance, offering free care finder services to the northern Perth community. The initiative is funded by the Australian Government's Primary Health Network Program, aiming to assist seniors in accessing essential aged care services.
Our care finder team is here to guide you through the complexities of aged care services, from gathering relevant information to navigating the application process, facilitating assessments and supporting you in selecting a suitable service provider. Suppose you're already receiving an aged care service, our team can help you transition to a facility that better meets your needs.
The services provided by Chung Wah CC's care finder team include, but are not limited to:
– Offering translation services, ensuring seamless communication.
– Representing applicants when liaising with My Aged Care and coordinating assessments.
– Assisting during assessments and offering support to applicants.
– Helping identify aged care service providers in the applicant's locality and filtering them based on specific requirements.
– Assisting with the completion of application forms and clarifying service agreements.
– Following up post the initiation of care services.
– Supporting the seniors in addressing other life challenges and accessing relevant assistance, like health concerns, housing difficulties, substance abuse, and referrals to social groups.
