11th June was a very special event in Buddhism. The fabulous care finder team from Chung Wah Community Care (Chung Wah CC) was invited to the Minh Quang Buddhist Temple, where they shared some helpful information about our care finder program with about a hundred members from the Vietnamese community.
Chung Wah CC is a proud member of the care finder initiative led by the WA Primary Health Alliance. We offer care finder services free of charge to the northern Perth community. The Australian Government's Primary Health Network Program funds this initiative which is specifically designed to assist vulnerable seniors who need support to access aged care services.
Our CWCC care finder team is always ready to help you understand information related to aged care services, guide you with the applications and evaluation process, and support you in choosing a service provider that's right for you. Suppose you're already receiving aged care services. In that case, we can help you find a new service provider that better suits your needs and smooth out the transitional process.
Our Chung Wah CC's care finder team is at your service with the following:
- Translation services to bridge the communication gap.
- Liaising with My Aged Care on your behalf to set up evaluations.
- Supporting you through evaluations.
- Assisting in finding the right aged care service providers in your area and helping you choose as per your needs.
- Assisting in form filling and understanding service agreements.
- Following up once you start receiving care services, and
- Help you overcome other life challenges and access the right support, such as physical and mental health issues, housing, homelessness, substance and alcohol abuse, and social group services referrals.
