Peace of Mind at CWCC: Community Nursing Station Supports Ageing with Confidence

In response to the challenges of an ageing society, Chung Wah Community Care (CWCC) is actively developing a new "Ageing Locally" model, delivering high-quality health management through professional, personalised community nursing services—all from the comfort of your familiar poses. On 31 March, CWCC's Community Nursing Station once again demonstrated the effectiveness of this approach during a health check-up session, which welcomed over a dozen participants, including local seniors and volunteers. The service received widespread praise from all who attended.

During this session, CWCC's professional nursing team provided one-on-one health assessments for each senior. Through basic checks such as blood pressure and heart rate monitoring, nurses were able to gain a comprehensive understanding of each individual's health status and offer tailored professional advice.

"My blood pressure has always been unstable," shared one participant. "The nurse not only recorded the data in detail but also taught me how to keep a blood pressure diary so my doctor can better understand my condition during follow-ups." For seniors with hypertension, the team designed customised daily monitoring plans. In cases of insomnia, practical advice was offered based on lifestyle habits to help improve sleep quality.

When one participant reported that shoulder pain affected their daily life, the nursing team immediately initiated the case management process. They helped the individual contact My Aged Care to reassess their service needs. They referred the case to a dedicated case manager, ensuring timely and appropriate support at home. This seamless and responsive service model reflects CWCC's commitment and professionalism in senior care.

CWCC is continuously expanding its services, guided by its clients' needs. During this event, the team carefully recorded requests for additional services such as blood glucose and cholesterol testing, incorporating them into future service improvement plans. To further enhance service quality, CWCC also plans to introduce a more standardised service labelling system, offering seniors a clearer and more professional health management experience.

"The nurses didn't just answer my health concerns—they also corrected some of my misconceptions about wellness," said another senior. "They reassured me about nail and skin health with professional advice, and I felt truly cared for."
