On March 11th, the care finder team from Chung Wah Community Care (CWCC), with the assistance of the Bayswater City Council, held a seminar on "Home Care and Elderly Services" at the Morley Community Centre. The seminar attracted over 50 community members. This event is part of the care finder Program.
Chung Wah Community Care has become a leading member of the WA Primary Health Alliance's care finder Program, offering free care finder services to the community in Perth. The care finder Program is funded by the Australian Government's Primary Health Network Program. This program aims to help seniors in need of care to access aged care services.
The CWCC care finder team can assist you in understanding information related to aged care services, applying, evaluating, and supporting you in choosing the right service provider. If you have already received aged care services, our team can help you find and switch to a more suitable new service provider. Our services include but are not limited to, providing translation services to facilitate communication between parties.
Communicating with My Aged Care on behalf of the applicant and arranging for assessments; participating in assessments and providing support to the applicant; helping to find aged care service providers in the applicant's area and assisting in filtering according to their requirements; assisting applicants in filling out forms and understanding service agreements. Following up with the applicant after receiving care services and helping the individual in dealing with other life difficulties and accessing related assistance, such as physical and mental health issues, housing difficulties and homelessness, substance and alcohol abuse, and referral to social group services.
