In the heart of Mental Health Week 2023, Chung Wah Community Care (Chung Wah CC) made its presence felt by partaking in the 'Picnic in the Park' events, organised by Helping Minds in the Northern and Southern regions. At these gatherings, Chung Wah CC set up stalls to raise awareness among non-English speaking community members about the services offered through the care finder program. As a proud participant in the WA Primary Health Alliance-led initiative, Chung Wah CC offers the care finder service free of charge to the community across Perth's North and South areas.
The care finder program, funded by the Australian Government's Primary Health Network Program, is designed to assist older adults in need of care to access aged care services. Our care finder team can guide you through understanding the information about aged care services, help with applications and assessments, and support you in choosing the right service provider. If you're already receiving aged care services, our team can assist in transitioning to a new provider that better suits your needs.
The suite of services provided by the Chung Wah CC care finder team includes but is not limited to:
– Translation services to facilitate communication
– Acting on behalf of clients to communicate with My Aged Care and arrange assessments
– Participation in assessments to provide support to applicants
– Assistance in locating aged care service providers in the client's area and helping with the selection process based on their preferences
– Aid in completing forms and understanding service agreements
– Follow-ups after the commencement of care services and
– Assistance in addressing other life challenges and accessing related support, such as physical and mental health issues, housing difficulties and homelessness, substance and alcohol abuse, and referral to social group services
