The care finder team from Chung Wah Community Care (Chung Wah CC) had the absolute pleasure of presenting the care finder program at the ISHAR Multicultural Women's Health Services community centre in Mirrabooka. This session was well received, with around 40 community members from non-English speaking backgrounds in attendance. Chung Wah CC is a proud member of the care finder initiative spearheaded by the WA Primary Health Alliance. This initiative provides free care finder services to communities in northern Perth. It is generously funded by the Australian Government's Primary Health Network Program. The program aims to support older individuals needing care to access essential aged care services.
Our committed care finder team assists with understanding aged care service-related information, application processes, assessments and providing guidance in choosing the most suitable service provider. For those already receiving aged care services, our team is also available to aid in finding and transitioning to a more suitable service provider.
The services that the Chung Wah CC care finder team provides are diverse and comprehensive, including, but not limited to:
– Offering translation services to promote communication,
– Facilitating communication with My Aged Care on behalf of applicants and arranging assessments,
– Actively participating in assessments while providing support to applicants,
– Assisting in finding aged care service providers in the applicant's area, following their requirements,
– Guiding applicants in filling out forms and understanding service agreements,
– Ensuring follow-ups after applicants start receiving care services, and
– Supporting senior individuals in managing life's obstacles and acquiring necessary assistance, be it physiological and mental health issues, housing difficulties and homelessness, substance and alcohol abuse, or social group service referrals.
